Excluding records governed by HIPAA, Your request for carrier records, such as an application or coverage election form, shall be submitted by You directly to the carrier which provided insurance coverage at the time the record was created.

Your request must comply with that carrier's rules, policies, procedures, and guidelines for record requests and your request may be subject to a copy charge
